Blood Testing for Fatigue
When fatigue does not improve with rest, targeted lab work uncovers what a basic checkup misses. Our comprehensive blood panel looks at the core systems that directly control how your body makes and uses energy.
Why a Routine Panel Is Not Enough
A standard checkup is designed to screen for disease, not to explain why you feel exhausted. It typically runs a CBC, a basic metabolic panel, and a single thyroid number — and if those fall inside broad reference ranges, the workup stops. That is how so many people end up hearing "your labs are normal" while running on empty.
Fatigue blood testing goes deeper. Instead of ruling out disease, it measures how well your oxygen delivery, thyroid hormone production, immune system, and cellular energy are actually functioning. The result is a clearer picture of why your energy is low, so your plan can be based on data instead of guesswork.
What the Fatigue Blood Panel Measures
Iron-related markers
Iron studies including ferritin and binding capacity measures evaluate for iron deficiency and the different forms of anemia. Ferritin reflects your stored iron, and it can be low long before a standard blood count flags a problem. When iron is low, your body cannot make enough healthy red blood cells — and oxygen delivery drops. That shows up as fatigue, weakness, shortness of breath with simple tasks, and brain fog. More on this at anemia and fatigue.
Complete thyroid workup
We run a complete thyroid workup, not just a screening number: regulatory hormones such as TSH alongside active thyroid hormones (free T3 and free T4), with thyroid antibodies added when clinically appropriate. TSH alone does not tell you whether your body is converting thyroid hormone into the active form your cells use. See thyroid fatigue for the fuller picture.
Lyme antibody immune markers
Immune markers screen for Lyme disease and related tick-borne infections that can drive long-lasting exhaustion. A lingering infection places a constant load on your immune and nervous systems, which can leave you wiped out after a normal day. Read more at Lyme disease fatigue.
Epstein-Barr virus indicators
EBV indicators point to lingering or reactivated viral stress tied to post-viral fatigue. Epstein-Barr virus can stay in the body long after the initial illness and keep the immune system in low-grade fight mode, draining energy and interfering with how cells produce it. See EBV fatigue.
CBC and metabolic markers
A complete blood count and key metabolic markers assess oxygen delivery, inflammation, and overall cellular energy production. These are the fundamentals — if oxygen is not reaching your tissues or inflammation is smoldering, no amount of rest fixes the fatigue.

Connecting the Markers to Your Symptoms
Numbers on their own do not treat anyone. What matters is whether the findings line up with your clinical picture: the afternoon crashes, the hair shedding, the cold hands, the sore throat that keeps coming back, the workout that now takes three days to recover from. We match the data to your story and rank what is actually driving the fatigue — because one patient may have low ferritin plus subclinical thyroid dysfunction, while another is dealing with a viral burden or an inflammatory pattern.
Who This Testing Is For
- You feel exhausted most days despite adequate sleep
- You were told your labs are normal but still feel unwell
- You have brain fog, low motivation, or slow exercise recovery
- You have a history of chronic stress, illness, or hormone-related symptoms
- You want an actual explanation of why your energy is low
What Happens After the Blood Draw
Your results are reviewed with you in detail, connected to your history and symptoms, and translated into a prioritized plan — what to address first, what to retest, and what to watch. You can see the full process on our how it works page, and if a blood draw is hard to fit in, most of this panel can run through remote testing across North Carolina with a draw at a lab near you.
