Spirometry: Lung Function & Oxygen Delivery Testing

Energy production starts with oxygen. Spirometry is a simple breathing test that measures how well your lungs move air in and out — clarifying whether your breathing is quietly limiting your stamina.

Why Oxygen Delivery Matters for Energy

Every cell in your body makes energy by burning fuel with oxygen. If oxygen delivery is compromised anywhere along the chain — low iron, weakened lung function, poor circulation — energy production drops and fatigue follows. You feel it as getting winded on stairs, needing to sit down after simple tasks, or hitting a wall much earlier in a workout than you used to.

The lungs are the entry point. With every inhale, oxygen crosses into your bloodstream and binds to hemoglobin for delivery to your tissues. If airflow is restricted or lung capacity is reduced, that hand-off happens under pressure — your body gets by at rest, then falls apart the moment demand rises.

That is why we treat oxygen delivery as a core part of a fatigue workup rather than an afterthought. Blood testing (see comprehensive blood testing) tells us whether your blood is carrying oxygen well; spirometry tells us whether your lungs are loading it properly in the first place.

What Spirometry Measures

  • Lung capacity — how much air your lungs can hold and move
  • Airflow speed — how quickly air moves in and out with each breath
  • Airway narrowing — helping detect narrowed airways or reduced lung performance
  • Breathing limits on activity — clarifying whether breathing is what stops you during exercise or daily tasks

If your lungs cannot load oxygen efficiently, your muscles and brain receive less of it — and no amount of rest or supplementation fixes a delivery problem. Used alongside other testing, spirometry helps determine whether reduced oxygen delivery is part of the reason you feel worn out.

What the Test Is Like

Spirometry is quick and done in the office. You breathe into a mouthpiece that records how much air you move and how fast, usually over several efforts so we get a reliable picture. There is no preparation, no needles, and no downtime — most patients are in and out of this part of the visit in minutes.

Results are interpreted in context. A normal spirometry reading is useful information too: it tells us oxygen delivery at the lungs is not the bottleneck, and we can move attention to iron status, thyroid function, or cellular energy production instead.

It is especially helpful when fatigue comes with shortness of breath, low stamina, a persistent cough, a smoking history, or suspected lung issues. In those cases, an honest picture of lung function changes the plan: it tells us whether breathing retraining, further medical evaluation, or other support should come first.

Common Signs Breathing May Be a Factor

  • Getting winded on stairs or carrying groceries
  • Needing longer recovery after mild activity than makes sense
  • A persistent cough or wheeze alongside the fatigue
  • History of smoking, asthma, or respiratory illness
  • Exercise tolerance that dropped noticeably without explanation

How It Fits Into a Fatigue Evaluation

Spirometry is one tool in the panel, ordered when the clinical picture calls for it — the right tests for the right patient, not everything for everyone. Paired with an EKG, it covers the two most common physical limits on stamina, heart and lungs; paired with blood work and the Organic Acids Test, it completes the picture of how oxygen and fuel turn into energy.

Since spirometry requires an in-office visit, it is the exception to our remote-friendly panel — but it can be scheduled alongside your other in-person visits at our Wilmington clinic. The full evaluation sequence is explained on our how it works page.
